We understand that circumstances may change. To ensure fair billing practices, our cancellation policy is designed as follows:

NO SHOW/CANCELLED APPOINTMENTS:

  • Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the practitioner's day that could have been filled by another patient. As such, we require 24 hours' notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours' notice, or miss their appointment, will be charged a cancellation fee of $50 to the card on file.

MEMBERSHIP CANCELLATION:

  • Patients must notify our office of their intention to discontinue their membership as soon as possible to avoid unnecessary charges. There will be no refunds given for membership fees that have already been collected.
  • Patients will be billed for the equivalent of the months' supply of medications that have been prescribed to them. This calculation is based on the number of months' worth of medication that has been dispensed or prescribed.
  • If medication refills have been authorized beyond what was dispensed, then the remaining refills will be cancelled.
Exceptions may be considered in cases of medical necessity or other extenuating circumstances. Please feel free to contact our office if you have any questions regarding this policy.
